Complete the Passport Application form by visiting  https://eservices.immigration.go.tz/online/passport

All passport applicants must complete all required online procedures before contacting the Embassy for submission and appointments. The following documentation must be uploaded when applying for New Passport, Stolen or Lost Passport and submitted with the application form to the High Commission:

New Passport (All Applicants must appear in person at the Tanzania High Commission for finger prints)

Requirements:

1. One (1) passport size photos to be uploaded online.
2. Original and Copy of the Old passport.
3. Original and Copy of the Birth certificate.
4. Birth Certificate or Affidavit of one of the parents.
5. Original and Copy of UK residence permit .
6. Marriage certificate for married women who have changed names
7. In case of changes on name(s), a deep poll
8. Passport Fee is $90 paid online while filling application.
9. Passport application supporting letter.
10. Two passport size photos

Stolen/Lost Passport

Requirements:

1. Police incident report
2. Newspaper advertisement
3. Letter stating reasons for lost/ stolen or damaged passport
4. Original and Copy of UK residence permit
5. Original and Copy of Birth certificate
6. Birth Certificate or Affidavit of one of the parent
7. One (1) passport size photos to be uploaded online
8. Fee is $ 250 lost for the first time and $ 350 lost for the second time or more
   * Fee is $ 150 for damaged Electronic passport
9.  Two passport size photos
